Job Overview

The Clinical Care Specialist is responsible for managing and coordinating care for clinically complex members. This role involves assessing patient needs, guiding treatment plans, and working closely with internal and external stakeholders to ensure optimal clinical outcomes. It is ideal for experienced nurses who are passionate about patient care coordination and healthcare innovation.

Key Responsibilities

• Conduct health assessments to identify patient risk factors
• Implement interventions to manage and reduce clinical risks
• Promote patient behavior change for better treatment adherence
• Authorize hospital admissions and treatments in line with protocols
• Assist members with clinical queries and healthcare guidance
• Monitor patient progress through follow-ups and ongoing engagement
• Coordinate integrated care, including referrals to social services
• Support members in understanding and maximizing their healthcare benefits
• Build and maintain relationships with internal and external stakeholders
• Refer cases to relevant departments for appropriate care delivery

Qualifications

Education

• Matric (Grade 12)
• Registered Nurse qualification
• Valid SANC registration

Experience

• Minimum 3 years’ clinical nursing experience post-qualification
• Managed care experience (advantageous)
• Knowledge of Discovery Health systems (advantageous)

Skills

• Strong clinical assessment and decision-making skills
• Excellent communication and interpersonal abilities
• Ability to manage complex cases and multiple stakeholders
• Customer-focused with strong problem-solving skills
• Organizational and planning skills
• Computer literacy (MS Office)
• Ability to adapt in a fast-paced environment
