Key Responsibilities:
✔ Psychosocial Assessments: Evaluate patients and their families to identify emotional, social, and cognitive needs.
✔ Counseling Services: Provide guidance and emotional support to patients, families, and significant others.
✔ Discharge Planning: Collaborate with the multidisciplinary healthcare team to create effective discharge plans that meet patient needs.
✔ Crisis Management: Assist patients in coping with new diagnoses, chronic illness, or terminal care decisions.
✔ Collaboration: Work with physicians, nurses, and other healthcare professionals to deliver comprehensive care.
Required Qualifications:
✔ Education: Graduate from an accredited Masters of Social Work (MSW) program.
✔ Experience: Minimum 2 years of hospital-based social work experience.
✔ Certifications: Active LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) certification is required for New Jersey positions, and preferred for Pennsylvania.
✔ Skills: Strong interpersonal skills and ability to handle sensitive issues with empathy and professionalism.
