Jefferson Health, a nationally ranked healthcare system serving patients in Philadelphia, Lehigh Valley, and Southern New Jersey, is looking for a compassionate Social Worker/MSW Relief to join our team at the Cherry Hill location.
Role Overview:
As a Social Worker/MSW, you will be responsible for providing essential social work services to patients and their families. You will assess and enhance the patient’s ability to cope with physical, emotional, and environmental challenges related to illness, convalescence, and discharge planning. This role involves working collaboratively with a multidisciplinary healthcare team to create effective discharge plans and ensure a smooth transition from hospital to home.
Key Responsibilities:
✔ Patient & Family Support: Provide emotional and practical support to patients and families dealing with the challenges of illness and hospitalization.
✔ Discharge Planning: Work closely with the healthcare team to establish and implement effective discharge plans, ensuring patients have the resources they need for post-hospital care.
✔ Crisis Intervention: Address the emotional needs of patients and families, including helping them navigate through difficult situations related to hospitalization and treatment.
✔ Collaboration: Collaborate with doctors, nurses, and other healthcare professionals to assess patient needs and contribute to their overall treatment plan.
✔ Documentation: Maintain accurate and up-to-date records of assessments, interventions, and progress related to the patient’s care and discharge.
Requirements:
✔ Master of Social Work (MSW) from an accredited program.
✔ Minimum of 2 years of hospital-based social work experience.
✔ Active LSW or LCSW (Licensed Social Worker or Licensed Clinical Social Worker) required for New Jersey positions and preferred for Pennsylvania positions.
✔ Strong communication skills to effectively engage with patients, families, and healthcare teams.
✔ Caring and compassionate demeanor with a commitment to improving the lives of patients and their families.
✔ Ability to handle crisis situations with professionalism and empathy.
