🔍 Purpose of the Role
The Case Officer plays a critical role in implementing and monitoring the Case Management Model (CMM). You will focus on adherence support for HIV clients on antiretroviral treatment (ART), ensuring continuity of care until they qualify for differentiated care models. This role blends community outreach, counselling, and administrative coordination to make a direct impact in the fight against HIV and TB.
🧾 Key Responsibilities

Capture and monitor ART patient information on the Case Management Model (CMM) Excel tool.

Track and record appointments for patients on ART for under 12 months, as well as high-risk clients (e.g. high viral load, pediatric, pre-ART, pregnant clients).

Perform daily telephonic follow-ups and coordinate linkage to Community Health Workers for clients who miss appointments.

Conduct home visits to patients requiring additional support or those lost to follow-up.

Send scheduled SMS reminders and respond professionally to messages from clients.

Refer patients to support groups based on their preferences.

Ensure continuous engagement with the care team to improve patient outcomes.
✅ Minimum Requirements

Matric / Grade 12 certificate
-
Enrolled Nurse Qualification (Advantageous; especially for pediatric streams)

Minimum 2 years of experience in HIV counselling, testing, and adherence support

Familiarity with index case tracing, adherence counselling, and ART protocols

Proficiency in English required; isiXhosa advantageous

Must have a clear criminal record
