Leveraging state-of-the-art telehealth systems, remote monitoring devices, and a dedicated care team, Discovery ensures continuity, quality, and comfort in every patient journey.

As part of our expansion strategy, we are building a dynamic team of Clinical Consultants (Registered Nurses) to drive excellence in our home-based care offering throughout 2025.

Key Purpose of the Role

As a Clinical Consultant, you will work independently to deliver patient-centred, evidence-based care directly in patients’ homes. You’ll manage everything from assessments to interventions and care coordination, using advanced technology to ensure seamless, real-time patient monitoring.

Key Responsibilities

• Perform holistic, patient-centred assessments and develop collaborative care plans.

• Administer treatments such as IV therapies, wound care, nebulizers, and pain management.

• Monitor and respond to emergency clinical situations with appropriate interventions.

• Coordinate multidisciplinary care involving therapy services, social support, or hospital referrals.

• Install, troubleshoot, and manage remote patient monitoring devices.

• Provide compassionate education and communication to patients and families about the programme.

• Ensure accurate documentation and compliance with privacy standards (POPIA).

• Support patient onboarding and post-admission logistics, including medication coordination.

• Be available for after-hours clinical support as needed.

Required Qualifications and Experience

• Bachelor’s Degree or Diploma in Nursing.

• Must be registered and in good standing with the South African Nursing Council (SANC).

• Minimum of 3 years’ experience in community nursing, emergency, or urgent care settings.

• Valid Basic Life Support (BLS) certification.

• Fully vaccinated for COVID-19.

• Proficient in Microsoft Office and digital tools.

• Own reliable transport and valid driver’s license.

Desirable Skills & Experience

• Advanced skills in IV insertion, wound care, and pain management.

• Experience in home-based care or interdisciplinary healthcare delivery.

• Exposure to remote patient monitoring systems.
