About the Hospital at Home Program:
Our Hospital at Home program offers medical aid members access to a variety of healthcare services in the comfort of their own homes. By combining telehealth, digital tools, and remote monitoring, we offer high-quality care that delivers superior clinical outcomes and ensures patients receive hospital-standard care at home.
We are looking for a Clinical Consultant to join our growing team and help ensure our patients receive the best possible care. This role involves delivering clinical care in patients’ homes while maintaining safety, providing education, and working closely with a multidisciplinary team.
Key Responsibilities:
✅ Autonomous Practitioner: Perform holistic assessments and create patient-centered care plans.
✅ Clinical Care: Provide interventions such as IV therapies, nebulizers, and syringe driver management.
✅ Emergency Response: Recognize clinical emergencies and respond appropriately.
✅ Communication: Maintain effective communication with patients, carers, and the healthcare team to ensure safe care transitions.
✅ Technology Integration: Set up and troubleshoot remote monitoring devices, ensuring they are well-maintained.
✅ Patient Onboarding: Facilitate the onboarding process for new patients, ensuring seamless care delivery.
✅ Clinical Documentation: Maintain accurate patient records and share information in line with privacy regulations.
✅ After-Hours Support: Provide on-call support to ensure continuous care.
Key Competencies:
✅ Strong clinical skills in community or emergency care.
✅ Excellent communication and team collaboration skills.
✅ High attention to detail with a commitment to delivering quality care.
✅ Ability to adapt quickly in a dynamic environment.
✅ Problem-solving capabilities to manage clinical risks effectively.
Experience & Qualifications:
✅ Bachelor’s Degree/Diploma in Nursing.
✅ Registered with SANC and practicing.
✅ Minimum of 3 years of experience in community, emergency, or urgent care settings.
✅ Valid Basic Life Support certification.
✅ Own transport and valid driver’s license.
Advantageous:
✅ Specialty in IV drip insertion, wound care, and pain management.
✅ Experience as a Home-Based Care Nurse.
