About Hospital at Home
Discovery Health’s Hospital at Home program provides medical aid members with an alternative to traditional hospital care. Our personalized care team brings hospital-level healthcare into the comfort of patients’ homes, utilizing cutting-edge telehealth capabilities, digital tools, and real-time remote monitoring devices. This initiative ensures seamless care, superior clinical outcomes, and patient satisfaction.
Key Responsibilities
As a Clinical Consultant, you will play an essential role in providing exceptional care in patients’ homes. Your responsibilities will include:

Autonomous Patient Care:
✔ Conduct holistic, patient-centered assessments.
✔ Collaborate with patients to develop shared care plans.
✔ Provide high-quality interventions such as IV therapies, nebulizer treatments, wound care, and more. -
Clinical Monitoring and Emergency Response:
✔ Recognize and respond to emergency clinical situations such as anaphylaxis and cardiac deterioration.
✔ Ensure the safe environment of care by assessing patient homes and mitigating risks. -
Multidisciplinary Collaboration:
✔ Liaise with medical teams, therapists, and other professionals to address patient needs.
✔ Ensure smooth handovers and continuity of care between team members.
✔ Coordinate the delivery of services in accordance with patient care plans. -
Technology Integration:
✔ Set up and monitor remote health devices and troubleshoot any issues.
✔ Maintain the devices and collaborate with vendors to resolve technical problems. -
Patient and Family Education:
✔ Educate patients and families on the Hospital at Home program, medical devices, and care procedures.
✔ Provide clear and sensitive communication regarding complex medical information. -
Documentation and Compliance:
✔ Maintain accurate patient records and ensure all data complies with POPIA regulations.
✔ Oversee client onboarding and documentation of treatments, ensuring all SOPs are followed. -
After-Hours Support:
✔ Provide on-call and after-hours clinical support as needed for patients admitted to the Hospital at Home program.
Required Qualifications and Experience
✔ Education: Bachelor’s Degree or Diploma in Nursing.
✔ Registration: Must be registered with SANC and actively practicing.
✔ Experience:
✔ At least 3+ years of experience as a Registered Nurse in community, emergency, or urgent care settings.
✔ Experience in home-based care or similar clinical environments is advantageous.
✔ Skills:
✔ Proficient in Microsoft Office and fully computer literate.
✔ Valid Basic Life Support (BLS) certification.
✔ Strong clinical skills, particularly in IV drip insertion, wound care, and pain management.
✔ Additional Requirements:
✔ Must have access to own transport for patient visits.
✔ Valid driver’s license and ability to travel between patient homes.
✔ Fully vaccinated for COVID-19.
