**Job Description:**
Habit Health, a leading nationwide healthcare provider specializing in rehabilitation, is seeking a dedicated Health Navigator for our Wellington team. As a Health Navigator, you will play a crucial role in supporting the implementation of new Integrated Care Pathways in the region, ensuring the best possible care for our clients.
**Responsibilities:**
– Collaborate with clients and physiotherapy/rehabilitation teams to track and enhance rehabilitation progress towards client outcomes, addressing additional concerns efficiently.
– Develop and maintain local relationships with clinical partners/providers such as Specialists, GP’s, and Primary Health Care providers.
– Address cultural needs sensitively and onboard clients onto the Behavioural Change Platform, explaining its role in their care and ECP journey.
– Coordinate service providers to execute management plans efficiently, optimizing treatment effectiveness.
– Maintain regular communication with clients and clinical teams to review progress towards client outcomes.

**Qualifications and Experience:**
– Strong ACC operational, medical, and policy knowledge related to the rehabilitation industry.
– Proficiency in Microsoft Office 365.
– Bachelor’s degree in Physiotherapy, Occupational Therapy, Social Work, or Psychology (desirable but not essential).
– Annual Practicing Certificate as a registered health professional (if appropriate).
– 3+ years of experience in rehabilitation.
