**Job Summary:**

As a Licensed Clinical Social Worker (LCSW) at St. Elizabeth’s primary care offices, you will report to the Social Work Manager and provide comprehensive care management services to patients. This includes conducting biopsychosocial assessments, collaborating with primary care physicians for diagnosis and treatment, and offering brief therapy and follow-up care. Your role will primarily focus on adult and geriatric patients with emotional disorders and mental illnesses within a primary care setting.

**Key Duties & Responsibilities:**

– Conduct comprehensive biopsychosocial assessments to establish care plans.

– Collaborate with primary care physicians to determine diagnosis and treatment plans.

– Provide short-term therapy modalities such as CBT, motivational interviewing, crisis intervention, etc.

– Initiate contact with patients within one business day of referral.

– Document all interactions accurately and in a timely manner using EPIC.

– Provide patient/family education, counseling, and social work interventions.

– Act as a liaison between patients/families and healthcare teams.

– Identify social determinants affecting patient care and refer patients for case management as needed.

– Evaluate progress on care plans and adjust as necessary.

– Initiate contact with discharged ER patients for further mental health needs.

– Participate in continuous education and maintain licensure requirements.

– Assist in evaluating patient outcomes and departmental success metrics.

**Qualifications:**

– Education: Minimum of MSW from a Council on Social Work Education accredited school.

– Experience: Minimum of 2+ years of clinical social work experience in mental health.

– Licenses and Certifications: KY – Licensed Clinical Social Worker (LCSW), Class D Driver’s License.
