Position Overview

As a Home Care Enrollment Specialist, you will guide families through the enrollment process for in-home nursing care under DSCC’s Home Care Program. This includes both waiver and non-waiver services. You’ll play a critical role in ensuring children with complex medical needs receive the care they deserve.

Work Details:

  • Schedule: Monday to Friday, 8:00 am – 4:30 pm

  • Work Type: Hybrid (on-site and remote)

  • Annual Salary: $52,000 – $63,000 (commensurate with experience)

Key Responsibilities

  • Coordinate and facilitate enrollment into DSCC Home Care Programs

  • Educate families on services, eligibility, and care coordination

  • Collect and evaluate medical and financial documentation

  • Conduct home assessments for safety and care suitability

  • Assist in preparing for hospital discharge by coordinating care services

  • Collaborate with hospital staff, providers, and DSCC Core/Connect teams

  • Provide discipline-based insight to DSCC and contribute to complex case evaluations

  • Promote interagency collaboration and community education

Required Qualifications

Candidates must meet one of the following:

  • Option 1: Master’s degree in social or behavioral sciences or a related health field, and current IL licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)

  • Option 2: Three (3) years of full-time experience in medical or social service agency settings
