**Job Summary:**
As a Licensed Clinical Social Worker (LCSW) at Florence, you’ll play a crucial role in our Social Work Department within primary care offices, providing essential care management services to patients. Working under the guidance of the Social Work Manager, you’ll conduct comprehensive biopsychosocial assessments, collaborate closely with primary care physicians, and deliver brief therapy interventions. This role involves continuous follow-up with patients throughout their therapy journey, mainly focusing on adult and geriatric patients dealing with emotional disorders and mental illnesses in a primary care setting. Your responsibilities include assessments, treatment planning, therapy sessions, case management, and ensuring patient education and coping support.

**Duties & Responsibilities:**
– Conduct face-to-face comprehensive biopsychosocial assessments and collaborate with primary care physicians to determine diagnoses and treatment plans.
– Develop and implement care plans based on thorough psychosocial assessments and environmental factors affecting patient health.
– Maintain timely communication with patients/families post-referral and document all interactions adhering to HIPAA guidelines.
– Provide a range of treatment modalities including short-term CBT, solution-focused therapy, motivational interviewing, crisis intervention, and more.
– Identify social determinants affecting patient care and coordinate referrals for case management as needed.
– Evaluate progress on care plans, initiate contact with discharged patients for further mental health needs, and document all interventions accurately.
– Act as a liaison between patients/families and healthcare teams, facilitating effective communication and continuity of care.
– Seek continuous improvement in service quality, engage in continuing education, and contribute to departmental evaluation processes.
**Qualifications:**
– Education: MSW from an accredited school recognized by the Council on Social Work Education.
– Experience: Minimum 2+ years of clinical social work experience in mental health.
– Licenses and Certifications: KY – Licensed Clinical Social Worker (LCSW), Class D Driver’s License.
– FLSA Status: Exempt
