In this role, you will be the key link between the client, their environment, and other agencies, ensuring they receive the proper care. Key Responsibilities
As a Social Worker, your role will include:
- Conducting Client Interviews: Engage with clients to understand their issues, assess their needs, and set goals for recovery or improvement.
- Creating Care Plans: Develop and document personalized care and treatment plans based on client interviews and insights from other professionals.
- Providing Emotional & Practical Support: Offer both practical assistance and emotional support to clients, ensuring they feel empowered and confident throughout their recovery process.
- Coordinating External Help: If needed, collaborate with other agencies such as youth care or social services to provide comprehensive support.
- Monitoring & Reporting: Track the progress of care plans, adjust them when necessary, and maintain accurate administrative records.
- Team Collaboration: Actively participate in team meetings, sharing vital information and updates to ensure the best care is delivered.
Who We’re Looking For
To be successful in this role, you should:
- Have a relevant degree in Social Work, Community Work, or a related field (HBO level or equivalent).
- Possess excellent communication skills and be comfortable working both independently and in a team.
- Be respectful and confident in providing support to clients, always ensuring their dignity is upheld.
- Have a strong understanding of how to identify risk factors and assist clients in making significant life changes.
- Be adaptable and comfortable working with diverse groups, from young people to adults with varying needs.
