Job Overview
Position: Social Worker/MSW – Adult Inpatient Behavioral Health Unit
The Social Worker/MSW will provide essential social work services to patients in our Adult Inpatient Behavioral Health Unit. This role involves working with patients and their families to assess and enhance their ability to cope with physical, emotional, and environmental challenges related to illness and recovery. The Social Worker will collaborate closely with the healthcare team to develop and implement comprehensive discharge plans, ensuring a seamless transition for patients from inpatient care.
Key Responsibilities
🔹 Patient Support & Counseling
- Conduct psychosocial assessments to evaluate the social, emotional, and cognitive well-being of patients.
- Provide counseling for patients, families, and significant others, assisting them in coping with new diagnoses, chronic illnesses, and terminal or palliative care.
- Offer guidance and emotional support to patients undergoing difficult health transitions.
🔹 Discharge Planning & Coordination
- Work collaboratively with the multidisciplinary healthcare team to establish and implement effective discharge plans for patients.
- Ensure that patients are adequately prepared for life after discharge, focusing on long-term recovery and well-being.
🔹 Documentation & Reporting
- Maintain accurate and detailed records of patient interactions, assessments, and progress.
- Prepare regular reports on patient outcomes, discharge planning, and psychosocial issues.
🔹 Education & Advocacy
- Educate patients and their families on coping strategies and the resources available to them.
- Advocate for the needs of patients, ensuring they have access to the necessary care and support services.
Minimum Qualifications
🔹 Educational Requirements
- Master’s of Social Work (MSW) degree from an accredited program is required.
🔹 Experience
- A minimum of 2 years of experience in hospital-based social work.
- Experience in behavioral health and working with patients in inpatient settings is preferred.
🔹 Certification & Licensing
- Active LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) required for New Jersey positions (preferred for Pennsylvania positions).
🔹 Skills & Competencies
- Excellent communication and counseling skills.
- Strong collaborative abilities to work within a multidisciplinary team.
- Knowledge of discharge planning and patient advocacy.
- Ability to work under minimal supervision and manage multiple tasks efficiently.
- Proficiency in medical documentation and reporting.
