About the Role
Jefferson Health, a nationally ranked healthcare provider, is seeking a Social Worker/MSW Relief to join their team at Cherry Hill, New Jersey. As a Social Worker, you will play a pivotal role in helping patients and their families navigate the physical, emotional, and environmental challenges associated with illness, recovery, and discharge planning.
This is a relief position, offering an opportunity to collaborate with a multidisciplinary healthcare team to enhance patient care and improve discharge outcomes. Your responsibilities will include conducting assessments, offering coping strategies, and establishing discharge plans for patients based on their specific needs.
Key Responsibilities
🩺 Patient & Family Support
- Provide social work services to patients, families, and significant others, focusing on emotional, physical, and environmental challenges.
- Assist patients in coping with illness, treatment, and recovery, ensuring a smoother transition to home care or long-term care.
🏥 Discharge Planning
- Collaborate with the multidisciplinary healthcare team to develop and implement effective discharge plans for patients.
- Ensure patients understand their treatment plans and provide them with the resources necessary for a successful recovery at home.
📑 Documentation & Reporting
- Maintain accurate records and case notes in line with hospital policies and regulations.
- Prepare reports for the team to help track patient progress and the effectiveness of discharge planning.
🔍 Assessment & Crisis Management
- Conduct assessments to determine the needs of patients and their families.
- Provide crisis intervention and support during emergency situations or acute care.
Required Qualifications
🎓 Educational Requirements
- Master of Social Work (MSW) from an accredited program.
📝 Certification Requirements
- Active LSW or LCSW required for New Jersey positions (preferred for Pennsylvania positions).
💼 Experience
- Minimum of 2 years of hospital-based social work experience.
- Previous experience in discharge planning or patient advocacy is highly beneficial.

- Strong communication skills and the ability to work effectively with patients, families, and multidisciplinary teams.
- Ability to problem-solve and manage stressful situations with professionalism and compassion.
