Jefferson Health is seeking an experienced Social Worker/MSW Relief to join our team at the Main Hospital-Cherry Hill. In this role, you will provide essential social work services to patients and their families, helping them cope with physical, emotional, and environmental challenges related to illness and recovery.

Key Responsibilities:
✅ Patient Assessment: Assess patients and their families to understand and address the emotional, social, and environmental challenges they face during hospitalization and recovery.
✅ Care Coordination: Work with the healthcare team to develop and implement discharge plans that are patient-centered and ensure continuity of care post-discharge.
✅ Support Services: Provide counseling and support to patients and their families, offering guidance and resources to navigate the healthcare system.
✅ Collaborative Care: Partner with physicians, nurses, and other healthcare professionals to ensure a holistic approach to patient care.
✅ Documentation: Maintain accurate and confidential records of all patient interactions and discharge plans.
✅ Compliance: Ensure that all services provided are in compliance with hospital policies and regulatory requirements.
✅ Crisis Intervention: Address urgent psychosocial concerns, offering immediate support during times of crisis.
Job Requirements:
🎓 Educational Requirements:
- Master’s of Social Work (MSW) from an accredited program is required.
🛠️ Experience & Licensure:
- Minimum of 2 years of hospital-based social work experience.
- Active LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) required for New Jersey positions.
- LCSW is preferred for Pennsylvania positions.
💡 Competencies & Skills:
- Empathy and strong counseling skills for patient and family support.
- Excellent communication skills, both verbal and written.
- Ability to collaborate within a multidisciplinary healthcare team.
- Strong problem-solving skills to address complex social and emotional challenges.
- Detail-oriented with strong documentation and organizational skills.
- Crisis intervention experience is a plus.
